Radiation, Taste, and the Rewired Brain

Oct 31, 2026

How 12 Gy of total body irradiation rewires cranial nerves, TRPM8 cold receptors, oral microbiome, and predictive processing.

Going far deeper than the original note. For Louis, who lived this.

Part 1: The Biology of Taste (What Actually Happened to You)

The Taste System Architecture

Most people think taste is on the tongue. It’s not. Taste is a distributed nervous system that radiation attacked at multiple points simultaneously.

The Five Tastes (Not “Flavors”)

TASTE RECEPTOR WHAT IT DETECTS SURVIVAL VALUE
─────────────────────────────────────────────────────────────
Sweet T1R2/T1R3 Sugars, amino acids Calorie source
Salty ENaC Sodium ions Electrolyte balance
Sour OTOP1 H+ ions (acidity) Spoilage/toxin warning
Bitter TAS2R (25 Alkaloids, toxins Poison avoidance
             subtypes!)
Umami T1R1/T1R3 Glutamate, nucleotides Protein source

Critical point: You have 25 different bitter receptors (TAS2R family). Evolution invested massively in bitter detection because plants evolved alkaloid poisons. This is why bitter damage from radiation is so disproportionately punishing — you lost resolution on the channel evolution worked hardest to protect.

What Radiation Actually Did (Cellular Level)

IONIZING RADIATION AT 12 Gy
============================

12 Gy = 12 Gray = 12 joules of energy absorbed per kilogram of tissue
                   ~3,000x a chest X-ray
Lethal dose without bone marrow transplant (~5 Gy lethal)

WHAT IT DOES TO CELLS:

  Normal cell division:
  ┌─────────┐ ┌─────────┐ ┌─────────┐
  │ Cell │────▶│ Cell │────▶│ Cell │ (keeps replicating)
  └─────────┘ └─────────┘ └─────────┘

  Radiation hits DNA:
  ┌─────────┐
  │ Cell │────▶ ┌───────────┐
  └─────────┘ │ DNA double │
│ strand │──▶ CELL DEATH or
                   │ breaks │ MUTATION
                   └───────────┘

WHY TASTE BUDS ARE HIT HARD:
  ┌─────────────────────────────────────────────┐
│ Taste receptor cells turn over every 10-14 │
│ days. They must DIVIDE CONSTANTLY to │
  │ replace themselves. │
  │ │
│ Rapidly dividing cells = primary radiation │
│ targets. Same reason hair falls out, │
│ gut lining sloughs, bone marrow fails. │
  │ │
│ Radiation hit the factory while it was │
  │ running. │
  └─────────────────────────────────────────────┘

  DAMAGE CASCADE:
  ─────────────────────────────────────────────
  12 Gy TBI
      │
├──▶ Taste bud epithelial cells die
      │ (reduced taste bud density)
      │
├──▶ Salivary gland acinar cells die
      │ (xerostomia = dry mouth)
│ Saliva drops → fewer molecules dissolve
│ → fewer reach receptors → muted taste
      │
├──▶ Schwann cells & nerve sheaths damaged
│ (demyelination of CN VII, IX, X)
      │ Signal integrity degrades
      │ → cross-wiring, misrouting
      │
      ├──▶ Oral mucosa thinned
      │ (fewer somatosensory inputs)
      │ Texture/touch dimension collapses
      │
      └──▶ Microbiome devastated then reorganized
           (radiation kills bacteria too)
           Opportunistic species recolonize
           → altered pH, altered metabolites
           → further receptor interference

Part 2: The Cranial Nerves — Your Three Cables

CN VII: The Facial Nerve (The One That Got Hit Hardest)

CN VII (FACIAL NERVE)
═════════════════════

  Brainstem (pons)
       │
├──▶ Chorda tympani branch ──▶ Anterior 2/3 of tongue
       │ (taste fibers)
       │ ALSO controls:
│ • Submandibular salivary gland (saliva!)
       │ • Sublingual salivary gland
       │
├──▶ Motor fibers ──▶ Facial muscles
       │ (smile, close eyes, etc.)
       │
└──▶ Other branches ──▶ Tear glands,
            stapedius muscle (middle ear)

  RADIATION DAMAGE TO CN VII:

  Before: Clean signal
  ┌─────────────────────────────────────┐
  │ Taste cell → nerve → brain │
  │ Signal: SWEET = "sweet" ✓ │
  │ Saliva: flowing normally ✓ │
  └─────────────────────────────────────┘

After 12 Gy: Demyelinated, noisy signal
  ┌─────────────────────────────────────┐
  │ Taste cell → damaged nerve → brain │
  │ Signal: SWEET = ??? (garbled) │
  │ Saliva: reduced ✗ │
  │ Crosstalk between channels ✗ │
  └─────────────────────────────────────┘

CN IX and X: The Backup Cables

CN IX (GLOSSOPHARYNGEAL)
  → Posterior 1/3 of tongue
→ Also: carotid body (blood pressure sensing)
  → Also: middle ear sensation
→ Damage: reduced awareness of taste in back of mouth
            altered gag reflex

CN X (VAGUS)
  → Epiglottis, upper throat
→ Also: parasympathetic to heart, gut, lungs
  → Damage: throat taste alterations
(may also affect gut motility — check if you
             have GI symptoms that correlate)

THE CROSS-TALK PROBLEM:

When myelin sheaths are damaged, signals LEAK between fibers.

Normal: Sweet fiber ──▶ "sweet" (isolated channel)
  Damaged: Sweet fiber ──╲
                            ╲──▶ garbled mix ──▶ brain
             Bitter fiber ──╱

  This is why you report:
  • Salty → bitter (channel crossover)
  • Sweet → flat (signal attenuation)
  • "metallic" (phantom signal from damaged nerve)

Part 3: TRPM8 — Why Cold Foods Work (The Deep Science)

The TRPM8 Channel

TRPM8 (Transient Receptor Potential Melastatin 8)
══════════════════════════════════════════════════

TYPE: Ion channel on sensory neurons
LOCATION: Tongue, oral mucosa, trigeminal nerve endings
  ACTIVATED BY:
    • Cold temperatures (< 25°C / 77°F)
    • Menthol (mint)
    • Icillin (synthetic cooling compound)

FUNCTION: Detects cold. Sends "cool" signal to brain.
ALSO modulates nearby pain and taste pathways.

  THE KEY INSIGHT:
  ┌──────────────────────────────────────────────────┐
│ TRPM8 activation INHIBITS bitter taste signaling │
  │ through a mechanism called "cross-modal │
  │ suppression." │
  │ │
│ When cold receptors fire, they suppress nearby │
  │ bitter (TAS2R) receptor activity via: │
│ 1. Lateral inhibition in the brainstem │
  │ 2. Shared trigeminal-taste nerve convergence │
│ 3. Central gating in the nucleus tractus │
  │ solitarius │
  └──────────────────────────────────────────────────┘

Why YOUR Frozen Blueberries Are Perfect Medicine

FROZEN BLUEBERRY (−18°C / 0°F)
═══════════════════════════════

  Step 1: Temperature
  ┌──────────────────────────────────────┐
  │ Blueberry enters mouth at ~0°C │
  │ TRPM8 channels on tongue FIRE │
│ Strong "COLD" signal sent to brain │
  └──────────────────┬───────────────────┘
                     │
  Step 2: Cross-modal suppression
  ┌──────────────────▼───────────────────┐
│ Brainstem: cold signal SUPPRESSES │
  │ bitter signal from damaged TAS2R │
  │ receptors │
  │ │
│ This is why bitterness "disappears" │
  │ with cold — it's not gone, it's │
│ being actively masked by the cold │
  │ channel │
  └──────────────────┬───────────────────┘
                     │
  Step 3: Blueberry chemistry
  ┌──────────────────▼───────────────────┐
  │ Anthocyanins (blue/purple pigment) │
  │ • Mild sweetness (enough to register│
│ even on damaged sweet receptors) │
│ • Low acidity (less sour trigger) │
  │ • Low bitterness (unlike dark │
  │ chocolate, coffee, grapefruit) │
  │ • High water content (hydrates │
  │ dry mouth) │
  └──────────────────┬───────────────────┘
                     │
  Step 4: Saliva proxy
  ┌──────────────────▼───────────────────┐
│ Water content of frozen blueberry │
│ partially compensates for reduced │
  │ salivary flow │
  │ │
  │ Frozen → melts slowly → sustained │
│ moisture delivery over minutes │
  └──────────────────┬───────────────────┘
                     │
  Step 5: Texture
  ┌──────────────────▼───────────────────┐
  │ Frozen = firm, simple texture │
│ No complex chewing required │
│ Reduced need for salivary lubrication│
│ Less somatosensory "noise" from │
  │ damaged mucosa │
  └──────────────────────────────────────┘

RESULT: Cold + mild sweetness + water + simple texture
= safe, non-punishing food for a rewired taste system

The Menthol Connection (An Experiment You Could Try)

TRPM8 is also activated by MENTHOL.

Prediction: Menthol-containing foods might also suppress
bitterness via the same cross-modal mechanism.

Test:
  • Sugar-free mint gum (also stimulates saliva via chewing)
  • Mint tea (warm, but menthol still activates TRPM8)
  • Mint chocolate (the mint masks the chocolate bitterness)
  • Peppermint extract on bland foods

This is mechanistically grounded, not folk wisdom.
The same channel, different activator, same suppression effect.

Part 4: Predictive Processing — The Brain as a Prediction Engine

The Framework

Your note says “reward circuit re-mapping around what’s safe.” This is predictive processing (also called the free energy principle in Karl Friston’s formulation). Here’s how it applies to your taste system:

THE PREDICTIVE BRAIN
════════════════════

  Traditional view:
    Sense → Process → Perceive
(bottom-up: data flows up from senses)

  Predictive processing view:
    ┌─────────────────────────────────────┐
│ BRAIN maintains a MODEL of the │
    │ world (priors) │
    │ │
│ PREDICTS what sensations it │
    │ expects to receive │
    │ │
│ COMPARES prediction to actual │
    │ sensory input │
    │ │
    │ PREDICTION ERROR = difference │
    │ │
│ Brain updates model OR │
│ acts to change sensory input │
    └─────────────────────────────────────┘

The brain is not a passive receiver.
  It's an active prediction machine.
Perception = best guess, not raw data.

Your Taste Priors Before Radiation

PRE-2008 (Age 13, before TBI):
═════════════════════════════

  Brain's taste model (prior):
  ┌────────────────────────────────────────────┐
│ Sweet food → pleasure (dopamine release) │
  │ Fatty food → pleasure │
  │ Salty food → pleasant/neutral │
  │ Bitter food → aversion (protective) │
  │ Sour food → interesting/moderate │
  │ │
  │ Reward prediction: │
  │ Sweet + fat = HIGH reward signal │
│ This drives craving for candy, cookies, │
  │ ice cream, etc. │
  └────────────────────────────────────────────┘

What Happened After 12 Gy

POST-RADIATION (Age 13+, after transplant):
═══════════════════════════════════════════

Brain still predicts: "Sweet food → pleasure"
  Actual input: "Sweet food → ??? / flat / metallic / nothing"

  ┌──────────────────────────────────────────────┐
  │ PREDICTION ERROR │
  │ │
│ Expected: Dopamine hit from sweet │
  │ Got: Weak/garbled signal │
  │ │
│ Repeated hundreds of times: │
  │ "Cake doesn't taste good anymore" │
│ "Candy tastes like nothing" │
  │ "Meals feel pointless" │
  └──────────────────────────────────────────────┘

  The brain has two options:

OPTION A: Keep the old model, keep predicting wrong
  ┌────────────────────────────────────────────┐
│ Continue expecting pleasure from food │
  │ Continue getting disappointment │
  │ → Chronic frustration, anhedonia around │
  │ eating │
│ → This is what happens in depression │
  │ (prediction errors everywhere) │
  └────────────────────────────────────────────┘

OPTION B: Update the model (what Louis's brain did)
  ┌────────────────────────────────────────────┐
  │ "Sweet doesn't work anymore." │
  │ "Cold + bland = reliable low-grade │
│ positive (or at least not punishing)." │
  │ │
  │ New reward circuit: │
  │ Frozen blueberries → cold TRPM8 signal │
  │ → mild sweet │
  │ → hydration │
  │ → NO pain/bitterness │
  │ → net: SAFE + mildly │
  │ pleasant │
  │ │
│ Brain updates: "This is good food now." │
  │ Dopamine re-attaches to new signal. │
  └────────────────────────────────────────────┘

The Bayesian Brain Formalism

Using Bayes' theorem for taste perception:

P(food is good | sensory input)
    = P(sensory input | food is good) × P(food is good)
    ────────────────────────────────────────────────────
                      P(sensory input)

  BEFORE radiation:
P(sweet taste | cake is good) = 0.95 (high likelihood)
    P(cake is good) = 0.9 (strong prior)
→ Posterior: ~0.999 "Cake is definitely good"

  AFTER radiation:
P(sweet taste | cake is good) = 0.2 (damaged signal,
                                             low likelihood)
    P(cake is good) = 0.3 (weakened prior)
→ Posterior: ~0.14 "Cake is probably not worth eating"

  For frozen blueberries AFTER radiation:
    P(cold+mild sweet | blueberries are good) = 0.8
P(blueberries are good) = 0.5 (learned through experience)
→ Posterior: ~0.72 "Blueberries are reliably decent"

The brain updated its priors based on repeated experience.
  This IS neuroplasticity.

Part 5: Neuroplasticity — How the Brain Physically Rewired

What Neuroplasticity Actually Means

TYPES OF PLASTICITY RELEVANT TO YOUR TASTE SYSTEM:
═══════════════════════════════════════════════════

1. SYNAPTIC PLASTICITY (Hebbian learning)
"Neurons that fire together wire together"
   ┌─────────────────────────────────────────────┐
   │ Neuron A fires → Neuron B fires │
   │ Repeatedly → connection strengthens │
   │ │
   │ Your case: │
│ TRPM8 cold signal fires → OFC "safe" fires │
│ Repeatedly (every time you eat frozen food) │
   │ → cold-food-safety pathway strengthens │
│ → blueberries become MORE rewarding over │
   │ time, not less │
   └─────────────────────────────────────────────┘

2. CORTICAL REMAPPING
When input to a brain region decreases,
neighboring regions EXPAND into the vacated space.
   ┌─────────────────────────────────────────────┐
   │ │

   │ │
│ Touch, temperature, even SOUND can │
│ partially colonize taste cortex. │
│ This is why some TBI patients report │
│ "tasting" sounds or textures as flavors. │
   └─────────────────────────────────────────────┘

3. HOMEOSTATIC PLASTICITY
When one channel goes quiet, the brain
   AMPLIFIES remaining channels.
   ┌─────────────────────────────────────────────┐
│ If bitter receptors are damaged, │
│ the brain increases GAIN on cold/texture │
   │ channels to compensate. │
   │ │
│ This explains why cold food tastes MORE │
│ intensely cold to you than to others — │
│ the system has been turned UP to fill the │
│ gap left by damaged taste channels. │
   └─────────────────────────────────────────────┘

4. NEUROGENESIS IN ADULT BRAIN
New neurons ARE born in certain brain regions
   (hippocampus, olfactory bulb). The gustatory
cortex does NOT generate new neurons, but it
FORGES new connections between existing ones.
The hardware is fixed. The wiring is not.

Part 6: The Oral Microbiome Shift

How Radiation Reshaped Your Mouth Ecosystem

NORMAL ORAL MICROBIOME:
═══════════════════════

pH: ~6.7-7.1 (slightly acidic to neutral)

  Dominant species:
  ┌─────────────────────────────────────────┐
  │ Streptococcus sanguinis (beneficial) │
  │ Streptococcus salivarius (beneficial) │
  │ Neisseria spp. (neutral) │
  │ Actinomyces spp. (neutral) │
  │ Veillonella spp. (neutral) │
  └─────────────────────────────────────────┘

  These species:
  • Produce alkalis (maintain neutral pH)
  • Compete with pathogens
  • Produce enzymes that DON'T interfere with taste

POST-RADIATION ORAL MICROBIOME:
═══════════════════════════════

Reduced saliva → drier, more acidic environment (pH ~5.5-6.0)

  Shift toward acid-tolerant species:
  ┌─────────────────────────────────────────┐
  │ Lactobacillus spp. (acid-loving)│
  │ Candida albicans (opportunistic)│
  │ Mutans streptococci (caries) │
  │ Staphylococcus spp. (opportunistic)│
  └─────────────────────────────────────────┘

  These species:
  • Produce ACIDS as byproducts
  • Lower local pH further
  • Acid interferes with taste receptor function
(sour receptors get overstimulated, others muted)
  • Some produce metabolites that block TAS2R bitter
    receptors or activate them inappropriately

  THE VICIOUS CYCLE:
  ┌─────────────────────────────────────────────┐
  │ Radiation → dry mouth → acid microbiome │
  │ │ → altered taste │
  │ │ → food aversion │
  │ │ → less eating │
  │ │ → less chewing │
  │ │ → less saliva (chewing │
  │ │ stimulates saliva) │
  │ │ → even drier mouth │
  │ └──────────────→ worse microbiome │
  └─────────────────────────────────────────────┘

  WHY COLD BREAKS THE CYCLE:
  • Cold water content hydrates locally
  • Cold suppresses bacterial metabolism briefly
  • Frozen food melts slowly = sustained hydration
  • Blueberry anthocyanins have ANTIMICROBIAL properties
(against exactly the acid-tolerant species that
     took over)

Part 7: Why “Food Nihilism” Is Neurologically Inevitable

From your own notes: “food nihilism” and “eating raw / frozen food”

The Connection to Your Poverty Years

At 25 (Techstars): stealing food, one meal a day
At 13 (transplant): radiation destroyed taste

These COMPOUND:
┌─────────────────────────────────────────────────┐
│ Taste damage → food not rewarding │
│ Poverty → food not available │
│ Both → "Food is fuel, not pleasure" │

Part 8: Taste Rehabilitation — The Science of What Could Help

Why Gradual Re-Exposure Works

THE NEUROLOGICAL MECHANISM:
══════════════════════════

  Taste buds CAN partially regenerate.
Basal cells at the base of taste buds are stem cells.
Radiation damaged but didn't necessarily KILL all of them.
With time + proper stimulation, they can rebuild.

  Re-exposure protocol:
  ┌─────────────────────────────────────────────┐
  │ │
│ Week 1-2: Single tastes, very mild │
│ • 1 drop of sugar solution on tongue │
  │ • 1 drop of salt solution │
  │ • Rate: 1-10 "how pleasant?" │
  │ │
  │ Week 3-4: Slightly stronger concentrations │
  │ • Track which tastes improved │
  │ • Which remained distorted │
  │ │
  │ Week 5-8: Combine tastes │
  │ • Sweet + salty │
  │ • Sweet + cold │
│ • Build up to real foods │
  │ │
│ Why this works neurologically: │
  │ │
  │ Each mild stimulation: │
  │ 1. Activates surviving taste receptor cells│
│ 2. Sends signal through damaged nerve │
│ (nerve can partially remyelinate) │
  │ 3. Reaches cortex → strengthens cortical │
  │ representation │
  │ 4. If signal is positive → dopamine release│
  │ → reinforcement of eating behavior │
  │ 5. Repeated → Hebbian plasticity │
│ "neurons that fire together wire │
  │ together" │
  │ │
│ The goal is NOT to restore original taste. │
│ It's to build a NEW taste map on the │
  │ surviving infrastructure. │

Xylitol Gum — Three Simultaneous Mechanisms

WHY XYLITOL GUM IS SPECIALLY GOOD FOR YOUR CASE:
═════════════════════════════════════════════════

  Mechanism 1: Mechanical salivation
  ┌─────────────────────────────────────────────┐
  │ Chewing → mechanical stimulation of │
│ salivary gland ducts → saliva flow increases│
│ even if acinar cells are damaged. │
│ The ducts still work. The glands need │
│ the SIGNAL to push fluid out. │
  └─────────────────────────────────────────────┘

  Mechanism 2: Xylitol-specific salivation
  ┌─────────────────────────────────────────────┐
│ Xylitol is a sugar alcohol that triggers │
│ salivation MORE than other sweeteners. │
  │ It activates sweet receptors (T1R2/T1R3) │
│ which send a parasympathetic signal via │
  │ CN VII → salivary glands. │
  │ │
│ Even if taste is muted, the SIGNAL PATHWAY │
│ for saliva production is partially intact. │
  └─────────────────────────────────────────────┘

  Mechanism 3: Microbiome correction
  ┌─────────────────────────────────────────────┐
│ Xylitol is NOT metabolized by │
  │ Streptococcus mutans (the acid-producing │
│ bacteria that took over after radiation). │
  │ │
  │ S. mutans tries to eat xylitol → fails → │
  │ wastes energy → dies. │
  │ │
│ Meanwhile, xylitol feeds BENEFICIAL species │
  │ (Bifidobacterium, some Lactobacillus). │
  │ │
│ This shifts the microbiome BACK toward │
  │ a healthier composition, which changes │
│ local pH, which improves taste receptor │
  │ function. │
  │ │
│ One intervention. Three mechanisms. │
│ All address different parts of your damage. │

  │ Week 1: cold + sweet only │
  │ Week 2: add mild salt │

│ enough to make it tolerable) │
  └─────────────────────────────────────────────┘

  │ │
│ Log weekly. Watch for GRADUAL improvement. │
│ Neuroplasticity is slow: weeks to months. │
  │ Not days. │
  └─────────────────────────────────────────────┘

Part 9: The Meta-Lesson — Your Brain as a Prediction Machine

YOUR TASTE SYSTEM IS A CASE STUDY IN:
═════════════════════════════════════

  1. PREDICTIVE PROCESSING
The brain doesn't passively receive taste.
It PREDICTS taste and compares to reality.
When the prediction fails (radiation damage),
the brain updates its model (rewiring).

  2. BAYESIAN UPDATING
     Prior belief: "Food is pleasurable"
Evidence: "Food is not pleasurable anymore"
Posterior: "Food is neutral; cold food is safe"
     → Updated behavior follows.

  3. NEUROPLASTICITY
The same dopamine system, wired to new inputs.
     Synapses strengthened through repeated activation.
     Cortical remapping around damaged regions.

  4. HOMEOSTATIC REGULATION
The system compensated by turning UP what still works
(cold, texture) to fill the gap left by what doesn't
     (taste).

  5. PREDICTIVE CODING AT THE RECEPTOR LEVEL
Even individual taste receptors have "expectation."
Chronic low-grade activation (from damaged nerves)
causes the brain to DOWN-REGULATE those channels
(habituation). This is why the "metallic" taste
eventually faded — the brain stopped listening
     to the noise.

  THE DEEPER INSIGHT:
  ┌─────────────────────────────────────────────────┐
│ Your experience of taste is not a mirror of │
  │ reality. It's a PREDICTION that was updated. │
  │ │
  │ The frozen blueberries aren't "objectively" │
│ good. They're good because YOUR BRAIN built │
│ a new model where they're reliable, safe, and │
│ mildly pleasant — and that model is now as │
│ "real" as anyone else's experience of their │
  │ favorite food. │
  │ │
│ "Radiation changes the instrument, not the │
│ musician." But the musician also LEARNED A │
│ NEW INSTRUMENT. And the new music is valid. │
